If you have long term care continuation or long term care conversion coverage, you may designate one additional person to receive notice of lapse of coverage due to nonpayment of premium. If the premium remains unpaid 30 days after it was due, we will send a notice to you and your chosen designee. We will not terminate your coverage until 30 days after we send that notice. If full retroactive premium is paid in that 30-day period, your coverage will not lapse. To avoid unintentional lapse of your plan due to non-payment of premium, you may designate another person, in addition to yourself, to be notified if your policy is in danger of lapsing. To appoint a designee, please complete the Appointment of Designee Due to Nonpayment of Premium form.

We can retroactively reinstate your coverage within five months of termination if your nonpayment resulted from Functional Incapacity or Severe Cognitive Impairment. You'll need to submit a request with evidence and pay retroactive premiums.

Reinstatement

Normally, if premium is not paid in a timely manner, your long term care coverage will terminate on the last day of the month for which premium has been paid. We will, however, retroactively reinstate coverage if both of the following conditions are met within 5 months of the date coverage was terminated.

Medical Inability to Pay

You, or someone on your behalf, submits a written request for long term care plan reinstatement that includes evidence demonstrating to our satisfaction that your failure to make timely payment of premium was caused by the fact that you were Functionally Incapacitated or had a Severe Cognitive Impairment at the time your premium payments were due.

Retroactive Payment in Full

You, or someone on your behalf, pays the full retroactive long term care premium due.
